The Routine Case: Key Steps
1. Find out what original documents your client has. Compare them with the list of proof that the SOS prefers. If you need more proof, make a list of what you still need. The Record of Effort form is a good place to keep a list of what you have and what you need.
2. If the client doesn't have picture ID, tell the client to start by getting their school records (transcripts). If they can get an original yearbook with a picture, that can also be helpful.
3. Use school records (and any other ID available) to get a birth certificate and proof of a social security number.
4. Help client gather documents that show where the client lives.
Bills in the client's name for that address
Letter on official letterhead confirming where client lives from parole officer or shelter if client lives in shelter
Proof that the client is living with and related to someone who pays bills at the address where the client lives.
Mail from a government agency or medical providers sent to the client at that address
5. Take all originaal documents plus $10.00 to Secretary of State Office unless you are eligible to have the fee waived.
