If your client is a U.S. citizen and has a Prison ID card then he or she needs the following:

1.    A second form of Identification  -   This can include original school records and yearbooks.   


2.    Proof of a social security number –  Get a  NUMI-lite print out from the Social Security office or use work records

3.    A certified  birth certificate –  This may cost a fee and your client will have to prove who they are.  

4.     Proof of residence in Michigan  -   parolees can ask their parole officer for a letter that states where they live.  This letter must be dated in the past 30 days.  


5.    $10.00 to pay at the Secretary of State office   If your client has no money,  he or she can also ask to have the fee waived.        
 
Important: if the name on the prison ID doesn't match other documents: 
If the name on the prison ID card is different than the client’s name on other records,  then the SOS will not accept the Prison ID as proof of identity. The client cannot get the prison ID card changed.
